By submitting this application I agree to:

• immediately notify Encana and the school board in writing should I change my educational institution, program, student status or study period
• provide information or documents as requested by Encana to verify any statement made in this application

I understand that:

• if I make a false or misleading statement in this application or fail to disclose information as requested by Encana, I may be denied financial assistance and/or be required to immediately repay all financial assistance received
• Encana is not responsible for securing my employment after I have completed my course of study

I consent to:

• the collection, use, retention, disclosure and exchange of the personal information and documents referred to in this application by and between Encana, post-secondary institutions and third-party providers who assist Encana with the scholarship program for the purpose of awarding discretionary educational scholarships; I understand Encana may retain this information so long as reasonable to fulfill these purposes; for further information, I will refer to Encana's Privacy Policy or contact Encana's privacy officer
• the use of my name and photo for promotional and reporting reasons, without further compensation from Encana

• the information given in this application is true and complete and I understand it is subject to audit
